Common manifestations of vitamin D deficiency are symmetric low back pain, proximal muscle weakness, muscle aches, and throbbing bone pain elicited with pressure over the sternum or tibia. A hydroxyvitamin D level should be obtained in patients with suspected vitamin D deficiency. Deficiency is defined as a serum hydroxyvitamin D level of less than 20 ng per mL 50 nmol per L , and insufficiency is defined as a serum hydroxyvitamin D level of 20 to 30 ng per mL 50 to 75 nmol per L. The goal of treatment is to normalize vitamin D levels to relieve symptoms and decrease the risk of fractures, falls, and other adverse health outcomes.
Vitamin D is essential for promoting calcium absorption in the intestine, maintaining serum calcium and phosphate concentrations, and allowing proper bone mineralization. Insufficient serum 25 hydroxyvitamin D 3 25OHD concentrations lead to increased PTH secretion, which in turn accelerates bone resorption, especially of the cortical bone. In addition, osteomalacia during its mild and early clinical course can be misdiagnosed as osteopenia and osteoporosis. The objective of this study was to evaluate the request of biochemical determination of 25OHD concentration in routine practice during the differential diagnosis and workup of individuals with osteoporosis. Individuals between 40 and 90 years of age who had received no previous treatment with antiresorptive medications were included. Blood tests were reviewed to assess whether the attending physician requested the determination of vitamin D concentrations, and if so, its value was collected. The Endocrine Society published guidelines for the evaluation, treatment and prevention of vitamin D deficiencies. Screening is recommended for individuals at risk for vitamin D deficiency. Population screening or screening of those not at risk is contraindicated. The investigators said these dosing levels may not protect children from all potential risks or provide all potential benefits. Whether these doses have a true, consistent, protective effect in these population subgroups is unknown. Obese children and adults should be given at least twice or three times the vitamin D requirements previously specified, as should all individuals taking anticonvulsant medications, glucocorticoids, antifungals such as ketoconazole and medications for AIDS. The investigators said limits may be exceeded in individuals with deficiencies but should not be exceeded without supervision from a health care professional.

We suggest that all adults who are vitamin D deficient be treated with 50, IU of vitamin D2 or vitamin D3 once a week for 8 wk or its equivalent of IU of vitamin D2 or vitamin D3 daily to achieve a blood level of 25(OH)D above 30 ng/ml, followed by maintenance therapy of – IU/d (2|⊕⊕⊕⊕).
